Bizbox, Inc. ("Bizbox," "we," "us") respects your privacy. This notice explains what personal information we collect when you use the "Talk to Our Sales Team" form on this website, why we collect it, and your rights under the Data Privacy Act of 2012 (RA 10173).
What we collect
When you submit this form, we collect:
- Contact details: first name, last name, work email, mobile number
- Professional details: your position/job title, your facility or company name, facility type, city or province
- Your inquiry: what you need help with, the Bizbox product(s) you're interested in, and your message
We do not ask for, and you should not include, any patient information, medical records, or other sensitive personal information about third parties in this form.
Why we collect it
We use this information only to:
- Identify who is reaching out and respond to your inquiry
- Route your inquiry to the right team (Sales, Support, or Partnerships) based on what you selected
- Follow up with information relevant to what you asked about (e.g. a product demo, a proposal, pricing)
We do not use this information for unrelated marketing without a separate opt-in, and we do not sell it to third parties.
Our legal basis
We process your information based on your consent (RA 10173, Sec. 12(a)) — given when you check the box and submit the form. You may withdraw consent at any time by contacting us; this won't affect any inquiry we've already responded to, but we will stop further processing.
Who sees it
Your inquiry is received by Bizbox's Sales team, and may be logged in our company mailbox (info@bizbox.ph), where the form is delivered by email. We do not share it with any party outside Bizbox except:
- A service provider who operates this tool on our behalf, under a confidentiality obligation (Microsoft Azure, which hosts this website and delivers the form to us by email, and Cloudflare, which checks the form for spam)
- When required by law or a valid legal order
How long we keep it
We retain inquiry submissions for 12 months from your last contact with us, after which it is securely deleted or anonymized, unless you become a client (in which case it becomes part of your account records under our client agreement) or the law requires us to keep it longer.
How we protect it
We apply reasonable organizational, physical, and technical security measures appropriate to this kind of data, including access controls limiting who can view submissions and encryption in transit (HTTPS) between your browser and our servers.
